How Scholarly Publishing and Peer Review Work

Scholarly publishing is the system that turns research claims, methods, data and interpretation into a public scholarly record that other researchers can inspect, test, cite, challenge, correct and build upon. Peer review is one of its central quality-control mechanisms—but it is neither a truth machine nor a guarantee of correctness.

Peer review does not certify that a paper is true. It asks whether the work is sufficiently clear, rigorous, relevant and accountable to enter scholarly conversation.

The distinction matters. Research knowledge advances through an ecosystem: study design, ethics, data collection, analysis, writing, editorial screening, peer review, revision, publication, replication, criticism, correction and sometimes retraction. Publication is therefore a checkpoint inside a much longer process of knowledge testing.

Modern scholarly publishing is also changing quickly. Preprints make work visible before journal review. Registered reports move part of peer review earlier, before results are known. Open peer review makes some reviewer reports public. Data and code sharing make claims more inspectable. Persistent identifiers improve provenance. Artificial intelligence can assist authors, reviewers and editors while creating new obligations around disclosure, confidentiality and responsibility.

1. Scholarly publishing begins before the manuscript

The strength of a paper is constrained by the research that came before it. Peer review cannot repair a fundamentally inappropriate study design, missing ethical approval, badly collected data or an analysis that cannot answer the stated question.

This is why good scholarly publishing begins with research design, reporting standards, data stewardship and ethical governance rather than with the journal submission portal.

2. The manuscript is a representation of the research

A study may have been conducted carefully and reported poorly. Another may be written elegantly while hiding weak methods. Reviewers therefore need to distinguish the quality of the underlying research from the quality of its presentation.

Scholarly publishing works best when manuscripts reveal enough about the study that readers can understand what was actually done.

3. Journal choice is part of the research route

Authors select journals by scope, audience, editorial standards, publication model, indexing, turnaround, reputation, accessibility, cost and fit with the field.

A prestigious journal with poor scope fit may be a worse destination than a specialist journal whose readers can actually use the work.

4. Scope is the first editorial gate

Editors ask whether the paper belongs in the journal before asking whether it is publishable in the abstract.

Scope protects reader expectations and reviewer effort. Sending every technically competent paper to review would overload the system with manuscripts the journal was never designed to publish.

5. Submission creates a formal editorial object

A scholarly submission usually includes manuscript files, figures, tables, supplementary material, authorship information, conflicts-of-interest disclosures, funding statements, ethics information, data-availability statements and cover correspondence.

These materials create the first formal publication packet. Incomplete disclosure can become a research-integrity issue later.

6. Technical checks happen before intellectual review

Editorial offices may check file completeness, word limits, figure quality, anonymisation requirements, reference style, ethics statements, trial registration, plagiarism screening and mandatory reporting items.

Technical screening protects peer reviewers from spending expert time on manuscripts that fail basic submission requirements.

7. Editorial triage is not peer review

An editor can reject a manuscript without external review when it is outside scope, insufficiently novel for the journal, methodologically unsuitable, inadequately reported or unlikely to interest the journal’s readership.

Desk rejection can feel abrupt to authors, but it is a legitimate allocation decision when editorial criteria are transparent.

8. The editor is not merely a messenger

Peer reviewers advise. Editors decide.

The editor weighs reviewer expertise, relevance of comments, journal standards, ethical issues, scope and sometimes conflicting reviews. Two positive reports do not automatically compel acceptance, and one negative report does not automatically justify rejection.

9. Reviewer selection is an expertise-matching problem

Editors seek reviewers with enough subject and methodological expertise to evaluate the manuscript. One reviewer may understand the biological mechanism while another evaluates statistics or clinical design.

No single reviewer has to know everything when the editorial team can assemble complementary expertise.

10. Reviewer availability is a real bottleneck

Peer review depends heavily on researchers donating or allocating expert time. Invitations are frequently declined because of workload, conflicts, poor fit or timing.

This means review speed is partly a coordination problem, not simply an editorial-efficiency problem.

11. Expertise is not enough; conflicts matter

A technically ideal reviewer may be unsuitable because of recent collaboration, direct competition, financial relationships, institutional proximity or personal conflict.

Reviewers should disclose conflicts that could compromise—or reasonably appear to compromise—their independence.

12. Confidentiality protects unpublished work

Manuscripts under confidential peer review contain ideas, data and findings that may not yet be public. Reviewers should not use that information for personal advantage or share it outside the authorised review process.

COPE’s ethical guidance for peer reviewers emphasises confidentiality, competing-interest disclosure, respectful review and accountability.

13. Review models differ

Each model changes incentives, accountability and confidentiality. Journals should describe their review model clearly rather than treating “peer reviewed” as a sufficient explanation.

14. Anonymity reduces some pressures and creates others

Anonymity can protect reviewers from retaliation and encourage frank criticism. It can also reduce visible accountability and make inappropriate behaviour harder to attribute.

Open review can increase transparency while potentially discouraging junior reviewers from criticising powerful authors. There is no perfect model independent of context.

15. A peer reviewer evaluates claims, not personalities

The review should address methods, evidence, analysis, interpretation, reporting and relevance.

Nationality, institutional prestige, seniority, gender, ideology or writing accent should not substitute for evaluation of the research.

16. Review begins with the research question

Is the question clear? Is it answerable with the chosen design? Does the paper claim more than the study was designed to establish?

A sophisticated review does not begin by hunting for small errors. It begins by asking whether the research architecture supports the central claim.

17. Methods are the engine room

Reviewers examine sampling, controls, inclusion and exclusion criteria, measurement, procedures, instruments, interventions, data handling and analytic plans.

If the methods are too vague to understand what happened, the paper is difficult to evaluate and difficult to reproduce.

18. Statistical review asks whether inference is justified

Reviewers may examine sample size, assumptions, multiple comparisons, model choice, missing data, effect sizes, uncertainty and whether reported tests match the stated design.

A statistically significant result can still be trivial, biased or methodologically inappropriate. Statistical significance is not a universal quality certificate.

19. Results should be separated from interpretation

The results section should report what was observed. The discussion interprets what those observations may mean.

Peer review often catches manuscripts where speculation is written as though it were measured fact.

20. Causal claims deserve special scrutiny

Observational association does not automatically establish causation. Reviewers ask whether design, controls, temporal order and alternative explanations justify causal language.

Often the correct editorial repair is not to reject the study but to narrow the claim.

21. Literature review should position, not merely decorate

Citations should show what is already known, where disagreement exists and how the present study changes the evidence landscape.

Reviewers may identify missing foundational literature, but should not use review power to demand irrelevant citations to their own work.

22. Citation coercion is an integrity problem

Reviewers and editors should not pressure authors to add citations merely to inflate personal, journal or institutional metrics.

Suggested citations should improve the scholarship, not the reviewer’s citation count.

23. Ethical review is part of publication review

Research involving humans, animals, sensitive data or other regulated activities can require ethics review, informed consent or institutional oversight.

Editors and reviewers do not replace ethics committees, but they can identify missing or inconsistent ethics information that must be resolved before publication.

24. Reporting guidelines improve inspectability

Different study types have specialised reporting guidelines—randomised trials, systematic reviews, observational studies, diagnostic studies and many others.

Reporting guidelines do not guarantee good research. They help authors reveal the information readers need to judge the research properly.

25. Reproducibility begins with enough information

A paper cannot be meaningfully reproduced if key methods, data transformations, parameters or materials are omitted.

Peer review increasingly evaluates whether code, data, protocols and supplementary materials are available when appropriate and lawful.

26. Data availability is a publication claim

“Data available on request” and “data publicly deposited” are different access states.

Journals need data-availability statements that accurately describe what readers can obtain, under what conditions and from which repository or custodian.

27. Open data has limits

Human-participant privacy, commercial restrictions, indigenous data governance, security, legal limits and contractual obligations can prevent unrestricted data release.

Open science should maximise inspectability without pretending every dataset can ethically be made public.

28. Code can be part of the evidence

When computational analysis produces published results, source code, scripts, environments and version information can be essential for verification.

A paper that reports complex analysis while preserving none of the computational pathway creates a fragile scholarly record.

29. Reviewer reports should distinguish fatal flaws from improvements

A useful review separates changes necessary to support the current claims from interesting additional research that would merely extend the work.

COPE guidance explicitly cautions reviewers against expanding a manuscript beyond its reasonable scope through unnecessary demands.

30. Constructive review is specific

“The methods are weak” is less useful than identifying which design assumption fails and what evidence would be needed to repair the problem.

Peer review should help the editor decide and help the author improve—even when the recommendation is rejection.

31. Tone is part of review ethics

Harshness is not rigour. Reviewers can be direct about serious weaknesses without humiliating authors or attacking competence.

Respectful criticism improves the scholarly system because authors can respond to the substance rather than defend themselves from unnecessary hostility.

32. Reviewers should not contact authors directly without permission

Direct contact can compromise confidentiality and bypass editorial oversight.

Questions and requests should usually travel through the journal’s editorial system unless journal policy explicitly provides another route.

33. Suspected misconduct belongs with the editor

If a reviewer suspects plagiarism, fabrication, duplicate submission, manipulated images or another integrity problem, the appropriate response is normally to notify the editor confidentially rather than conduct an independent investigation.

Research-integrity investigations require procedural fairness and institutional authority.

34. The editor synthesises conflicting reviews

One reviewer may request rejection while another recommends minor revision. The editor asks why.

Does one reviewer have relevant methodological expertise the other lacks? Are they interpreting the journal’s novelty threshold differently? Is a third opinion needed? Editorial judgement exists precisely because review reports are evidence, not votes.

35. Decisions have several forms

Terminology varies by journal. The important thing is that authors understand what decision was made and what work is required next.

36. Major revision is not acceptance

A major-revision decision usually means the paper remains under evaluation. The revised version may return to the same reviewers, different reviewers or the editor alone.

Authors should not assume that completing every requested change automatically guarantees publication.

37. The response letter is part of scholarly argument

Authors should address reviewer comments systematically, state what changed, identify where the change appears and explain respectfully when they disagree.

A response such as “done” provides less editorial confidence than a traceable explanation.

38. Authors are allowed to disagree

Peer review is not obedience. Reviewers can misunderstand a method or request a change that would weaken the paper.

Authors should explain disagreements with evidence and logic rather than treating reviewer comments as orders or dismissing them emotionally.

39. Revision should not create hidden new claims

Adding analyses, datasets or interpretations in response to review can materially change the paper.

Editors may need to reconsider whether the revised manuscript now requires additional review or new disclosures.

40. Acceptance is a transition into production

After acceptance, the manuscript moves through copyediting, typesetting, metadata creation, proofing, author corrections, licensing and final publication.

New substantive research claims should not be introduced casually at proof stage because peer review has already evaluated an earlier manuscript state.

41. The version of record anchors citation

Once formally published, the version of record becomes the journal-controlled edition that readers cite and correction systems update.

Earlier preprints and accepted manuscripts may remain valuable, but the publishing ecosystem needs clear relationships between versions.

42. Peer review can itself become a published object

Some journals publish reviewer reports, author responses and decision letters alongside the article.

Crossref supports registration of peer-review metadata and relationships connecting a review to the item it evaluates, allowing peer review to become a more visible and citable part of the scholarly record.

43. Review metadata strengthens provenance

Structured review metadata can include review date, contributors, type, recommendation, licence and a relationship such as isReviewOf.

This transforms peer review from an invisible editorial event into a traceable scholarly object when journal policy supports transparency.

44. Preprints move dissemination before journal review

A preprint allows authors to make a manuscript publicly available before formal peer review is complete.

This accelerates communication but changes reader responsibility. A preprint should not be represented as though it has passed the same journal review process as a version of record.

45. Preprints can receive public review

Researchers can comment, critique and replicate preprints before or alongside journal submission.

The boundary between prepublication and postpublication review is therefore becoming more fluid.

46. Registered reports move peer review earlier

Registered reports evaluate the research question and methodology before results are known. If the protocol passes review, the journal may offer in-principle acceptance contingent on authors following the approved plan and meeting publication conditions.

This can reduce incentives to favour surprising positive results over rigorous negative or null findings.

47. Publication bias distorts the visible literature

Studies with statistically significant or exciting findings can be more likely to be submitted, accepted or publicised than studies with null results.

The published literature can therefore misrepresent the full research landscape even when individual published papers are correctly reported.

48. Negative results can be scientifically valuable

A well-designed study that finds no meaningful effect can constrain theory, prevent duplicated effort and improve meta-analysis.

Scholarly publishing serves knowledge poorly if novelty consistently outranks methodological quality.

49. Replication is postpublication peer review by experiment

A finding becomes stronger when independent researchers can reproduce or replicate it under appropriate conditions.

Peer reviewers evaluate plausibility before publication. Replication tests robustness in the world after publication.

50. Citations are not votes of truth

A highly cited paper can be influential, controversial, foundational, flawed or frequently criticised.

Citation count measures attention and reuse more directly than correctness.

51. Corrections are part of scholarly self-repair

Published papers can contain errors that do not invalidate the entire work. Corrections can repair figures, text, data or interpretation while preserving the publication identity and historical record.

A healthy scholarly system treats visible correction as evidence of governance rather than automatic disgrace.

52. Retraction is a stronger status

Retraction tells readers that a paper should not be relied upon because major integrity or reliability problems undermine the work.

The retracted article generally remains identifiable so citations and the history of the problem can still be resolved.

53. Retraction is not deletion

Deleting a paper entirely can erase the evidence that a problematic publication existed.

Scholarly publishing therefore relies on status metadata, notices and linked correction records to preserve both accountability and citation continuity.

54. Expressions of concern represent unresolved risk

Editors may sometimes alert readers to serious concerns while an investigation remains incomplete.

This intermediate status prevents the false choice between saying nothing and retracting before sufficient evidence exists.

55. Appeals need procedural boundaries

Journals may allow authors to appeal decisions when they believe review was materially flawed, conflicted or based on a misunderstanding.

An appeal should identify substantive reasons, not merely disagreement with rejection. Editorial independence includes the ability to maintain a reasoned decision.

56. Complaints and appeals are different

An appeal challenges the scholarly decision. A complaint may concern process, delay, reviewer conduct, conflict of interest or editorial behaviour.

Separating these routes helps journals respond to the right problem.

57. Predatory or pseudo-journals imitate scholarly signals

Some organisations present themselves as scholarly journals while providing little or no genuine peer review, using misleading titles, false claims of indexing or aggressive fee practices.

ICMJE’s current recommendations warn authors about journals that claim scholarly standards without actually functioning as legitimate peer-reviewed publications.

58. A journal website should reveal its process

Transparent journals explain peer-review model, editorial board, fees, ownership, conflicts policy, corrections policy, ethics standards and contact information.

Opacity about basic governance should lower confidence before submission.

59. Publication fees do not automatically mean poor quality

Many legitimate open-access journals charge article processing fees. Many subscription journals charge other publication fees. Business model alone does not determine scholarly integrity.

The important questions are transparency, editorial independence, review quality, governance and whether payment influences acceptance improperly.

60. Editorial independence protects the scholarly decision

Journal owners and commercial teams should not dictate individual acceptance decisions because a paper is financially attractive, politically convenient or strategically useful.

Editorial independence creates a boundary between publishing economics and scholarly judgement.

61. Peer review has known limitations

These limitations are reasons to improve peer review—not reasons to pretend prepublication review has no value.

63. Artificial intelligence changes authorship questions

AI can assist drafting, language editing, coding, literature screening and analysis. It cannot take responsibility for the integrity of the work in the human sense required of scholarly authors.

Current ICMJE recommendations, updated in January 2026, include a dedicated section on responsible use of AI in publishing and continue to place accountability on human authors and editors.

64. AI should not be listed as an author

Authorship involves accountability, approval, disclosure and responsibility that a generative system cannot independently fulfil.

Human authors remain responsible for verifying AI-assisted content, sources, analysis and language before submission.

65. Reviewers face a confidentiality problem with AI tools

Uploading a confidential manuscript into an external AI service can expose unpublished content to systems outside the journal’s authorised workflow.

Reviewers should follow the journal’s current AI and confidentiality policy before using external tools on manuscripts under review.

66. AI can assist peer review inside governed systems

Editorial platforms can use controlled automation to check reporting completeness, references, image anomalies, statistics, conflicts, plagiarism signals and reviewer matching.

The safest role is triage and anomaly detection, with accountable human editors retaining the publication decision.

67. AI-generated review text still belongs to the reviewer

If policy permits AI assistance, the human reviewer remains responsible for the accuracy, tone, confidentiality and scholarly judgement in the submitted report.

Automation cannot absorb reviewer accountability.

68. Peer-review archives matter

Journals may preserve original submissions, reviews, revisions and correspondence because questions about a publication can arise years later.

ICMJE’s current recommendations advise journals to retain records of published manuscripts and their review history for at least several years and potentially longer depending on regulation and policy.

69. Review archives support integrity investigations

If allegations arise after publication, editors may need to reconstruct what authors submitted, what reviewers raised, what changed during revision and what evidence was available at acceptance.

The peer-review archive therefore becomes part of the provenance of the scholarly record.

70. Confidentiality and archival retention must coexist

Preserving editorial records does not mean making them public automatically.

Journals need retention, access and disclosure policies that respect reviewer confidentiality while preserving enough evidence for future governance.

71. The peer-review report is itself a scholarly contribution

A careful review can improve methods, clarify claims, identify missing literature and prevent unsupported conclusions from entering publication.

Recognition systems increasingly attempt to make this invisible labour more visible without compromising journal confidentiality policies.

72. Good reviewer training strengthens the system

Researchers are often expected to review manuscripts before receiving formal training in review ethics, statistics, bias, constructive feedback or editorial decision-making.

COPE’s reviewer guidance exists partly because ethical peer review is a professional skill, not an automatic by-product of subject expertise.

73. Early-career reviewers can contribute with supervision

Mentored review can train future reviewers, but the journal should be informed when another person contributes to a confidential review.

Unacknowledged delegation breaches accountability and can violate confidentiality.

74. Diversity improves coverage of blind spots

Reviewer pools dominated by one geography, institution type, discipline or career stage can miss contextual issues visible to others.

Diversity is not a substitute for expertise. It broadens the perspectives through which expertise is applied.

75. International review needs sensitivity to context

Methods, resources and reporting conventions can differ across settings. A reviewer should distinguish genuine methodological weakness from mere difference from the reviewer’s local practice.

Scientific standards should be rigorous without assuming one institutional environment is the universal default.

76. Language quality and scientific quality are different

Authors writing in a second language may present strong research in imperfect prose.

Reviewers should distinguish language clarity issues from scientific weakness and avoid demeaning comments about linguistic background.

77. Peer review should not become unpaid rewriting

Reviewers should identify problems and suggest improvements, not rewrite the manuscript sentence by sentence unless such intervention is genuinely necessary and within scope.

The paper remains the authors’ work.

78. Editorial speed must not erase due process

Fast publication can be valuable, especially during rapidly moving research areas. Speed becomes dangerous when reviewer expertise, conflict checks, ethical review or author response are compressed below meaningful thresholds.

Efficiency should reduce administrative delay, not intellectual scrutiny.

79. Slow review also has costs

Months of unexplained delay can harm researchers, slow knowledge transfer and leave important findings unavailable.

ICMJE recommends timeliness and responsiveness from journals and peer reviewers as part of responsible editorial process.

80. Transparent process builds trust

Readers and authors should be able to understand a journal’s peer-review model, editorial ownership, conflicts policy, correction policy, fees and publication ethics.

Transparency does not make a journal infallible. It makes the governance inspectable.

Science and scholarship advance because claims are not supposed to remain protected by the authority of the person who made them. Methods are exposed. Evidence is inspected. Reviewers challenge assumptions. Readers criticise. Other researchers attempt replication. Errors become corrigible.

The publication process is therefore not a ceremony that converts uncertainty into certainty. It is a system for making claims increasingly inspectable.

The strongest scholarly record is not one that never changes. It is one in which evidence, review, disagreement and correction remain visible enough for knowledge to improve.

86. Why scholarly publishing matters to civilisation

Modern civilisation depends on knowledge that no single person can independently verify from first principles. Medicine, engineering, climate science, economics, computing and public policy all rely on communities of expertise.

Scholarly publishing is one of the institutions that makes this distributed expertise legible. It records who claimed what, how they investigated it, who reviewed it, what was published, what was later challenged, and which parts of the record survived further scrutiny.
