Did you know? Polytechnic medical checks are not one identical requirement for every course and applicant. Start with the chosen institution’s official instructions: Nanyang Polytechnic’s Medical requirements page and Singapore Polytechnic’s enrolment page, Task 2, show why the exact course matters.
Match the course, intake and applicant category before arranging a check. Singapore Polytechnic distinguishes colour-vision testing for specified courses from its medical-examination instructions. Republic Polytechnic’s SC/PR enrolment guide similarly directs students to check whether their course needs a medical examination or colour-vision test.
Your immediate task is to identify the applicable requirement, download the current institution-specific form and check the submission deadline in your offer instructions. If a condition or requirement is unclear, contact admissions before making assumptions. This guide locates administrative information; it does not assess medical fitness or determine admission.

Practical checks before you act
Read the exact course row before drawing conclusions
Use the full course name and code from your application or offer. Read the relevant heading and any notes attached to the row. NYP’s page distinguishes different requirements and encourages applicants with the listed conditions to contact the institution for further information.
A statement that someone may face difficulties is not a diagnosis you should make from a webpage. Take a specific question to admissions and, where required, the professional completing the assessment. Do not declare yourself ineligible solely because an online discussion mentions a condition with a similar name.
Bring the institution’s current form to the appointment
SP expressly tells students to use its colour-vision form rather than another form or medical memo. RP’s guide instructs applicable students to obtain the relevant form, have it endorsed and upload it through the specified enrolment step. Read those instructions before arranging an appointment.
Keep the document title and version with your checklist. Ask which parts the student completes, which parts require professional endorsement and how the final document is submitted. This can prevent an avoidable second visit caused by bringing a generic form that does not answer the institution’s questions.
Keep applicability and deadlines together
Make one row per required document: who needs it, the current form, where to submit it and the deadline in your enrolment instructions. If the instructions refer to more than one recipient or process, track each separately. Completing one upload is not proof that every task is finished.
For a parent helping with paperwork, agree on who will check the student portal and official email. Save submission acknowledgements without circulating medical documents in group chats. If the deadline is approaching and an appointment or report is delayed, contact admissions promptly through its published route.
Ask for clarification without assuming an outcome
A useful enquiry names the course, intake and the unclear requirement. Ask what assessment or supporting document is needed and where the institution wants it sent. Follow its instructions on disclosure; do not include a full medical history in an initial message unless requested through an appropriate channel.
Keep three decisions distinct: academic eligibility, completion of the required medical assessment and the institution’s final admission or enrolment decision. A test appointment is not an offer, and an offer does not remove outstanding conditions. The existing enrolment guide below covers the wider new-student process.
