Failure Mode and Effects Analysis, or FMEA, is a proactive method for examining how a healthcare process could fail before an adverse event occurs.
Instead of waiting for an incident and then investigating backward, FMEA maps a process forward, asks where failure could occur, considers possible consequences and identifies safeguards that can reduce risk.
FMEA Begins With a Defined Process
The team first chooses a process with clear boundaries, such as medication administration, specimen handling, patient transfer or another safety-critical workflow.
Map the Steps Before Judging Them
Breaking the process into actual steps helps reveal handoffs, dependencies and assumptions that can otherwise remain invisible.
Identify Failure Modes
For each step, the team asks how it could fail: omitted action, wrong information, delay, incorrect identification, equipment problem, communication failure or another deviation.
Consider Effects and Existing Controls
The analysis asks what could happen if the failure reaches the patient and which safeguards already prevent, detect or contain it.
Prioritisation Should Lead to Redesign
The value of FMEA is not the worksheet itself. High-priority vulnerabilities should produce stronger controls, clearer ownership or redesigned processes.
