Current ISTH guidance recommends obtaining ADAMTS13 activity before plasma exchange or blood products when possible, but starting plasma exchange and corticosteroids without waiting for the result when clinical suspicion is high. Caplacizumab can also be considered early in the appropriate setting.

1. The Owner Is ADAMTS13-Deficient Microvascular Thrombosis
DIC owns systemic coagulation activation; HIT owns heparin-triggered platelet activation. TTP owns severe ADAMTS13 deficiency causing platelet-rich microvascular thrombosis with microangiopathic haemolysis.
2. The Blood Film Shows Mechanical Red-Cell Injury
Schistocytes, raised LDH, reduced haptoglobin and reticulocytosis can support microangiopathic haemolysis. The pattern helps identify thrombotic microangiopathy but is not unique to TTP.
3. Platelet Count Is a Disease-Activity Receipt
Severe thrombocytopenia is common and platelet recovery is an important treatment response, but organ injury and ADAMTS13 activity remain separate objects.
4. Neurological Symptoms Can Fluctuate
Headache, confusion, focal deficits, seizures or reduced consciousness may occur as microthrombi affect cerebral circulation. A normal scan does not exclude microvascular disease.
5. Cardiac Injury Can Be Hidden
Troponin elevation, arrhythmia or heart failure can occur from myocardial microvascular injury. Cardiac monitoring therefore belongs in severe presentations.
6. Plasma Exchange Replaces What Is Missing and Removes What Is Harmful
Therapeutic plasma exchange both supplies functional ADAMTS13 and removes circulating autoantibodies and other plasma factors. It remains a core emergency treatment in immune TTP.
7. Corticosteroids Target the Autoimmune Driver
Immunosuppression reduces ongoing autoantibody production. Rituximab may be added in acute treatment or relapse-prevention strategies depending on disease course and specialist judgment.
8. Caplacizumab Targets Platelet–von Willebrand Factor Interaction
Caplacizumab can reduce ongoing microvascular platelet adhesion while the autoimmune process is brought under control. It changes thrombotic activity quickly but does not eliminate the underlying anti-ADAMTS13 autoimmunity by itself.
9. ADAMTS13 Is a Diagnostic and Long-Term Monitoring Object
Severe deficiency strongly supports TTP. During remission, persistently or recurrently low ADAMTS13 can identify relapse risk and inform pre-emptive immunosuppressive strategies.
10. TTP and DIC Must Stay Separate
The DIC Web owns consumption coagulopathy driven by systemic coagulation activation. TTP usually has a different coagulation profile and a fundamentally different treatment system.
11. Platelet Transfusion Is Not a Routine Reflex
Because platelet-rich microthrombi are central to TTP pathophysiology, platelet transfusion is generally reserved for compelling bleeding or procedural indications rather than used simply because the count is low.
12. Remission Is More Than a Normal Platelet Count
Clinical remission includes sustained platelet recovery and resolution of haemolysis/organ injury. Immunological remission and ADAMTS13 recovery may lag behind.
13. Relapse Is a Longitudinal Disease State
Patients can relapse months or years later. Education about symptoms, rapid access to haematology and periodic ADAMTS13 monitoring can reduce delay in future episodes.
14. Evidence, Uncertainty and Correction
ISTH’s 2025 focused update kept the core immune-TTP recommendations unchanged while updating congenital TTP guidance. The correction loop is TMA signal → high/low TTP probability → ADAMTS13 sample → urgent treatment when high probability → platelet/haemolysis/organ receipt → ADAMTS13 result → refine diagnosis and therapy → remission surveillance.
15. RFE: Did We Stop Microvascular Thrombosis Before the Brain, Heart or Kidneys Paid the Price?
The Medicine RFE asks whether timely, evidence-grounded and ethically authorised help reaches the human and improves outcomes without preventable harm. In iTTP, success means treatment was not delayed for laboratory certainty, microvascular thrombosis stopped, organ injury was followed, immune disease was suppressed and relapse risk remained visible after discharge.
