A care pathway is a structured route through healthcare that links assessment, diagnosis, treatment, monitoring, handover, recovery and follow-up around a defined patient problem or clinical journey.
Its purpose is to reduce fragmentation. A patient should not experience each encounter as an isolated event when the underlying health problem is one continuous story.

Pathways Reduce Lost State
