Wait, What? “Lipase” Is Not One Perfect Pancreas Signal

Vomiting, abdominal pain, poor appetite and lethargy can occur with pancreatitis—but also with many gastrointestinal, hepatic, renal and systemic disorders.

Likewise, some serum lipase activity assays detect enzymes from tissues other than the pancreas. The most useful pancreatic tests are therefore those validated for the specific biological question being asked.

Primary Entry — The Pancreas Has More Than One Job

The endocrine pancreas regulates blood glucose through hormones such as insulin and glucagon. The exocrine pancreas makes digestive enzymes and bicarbonate-rich fluid.

This page focuses on the exocrine pancreas: inflammation and loss of digestive secretory capacity.

Part 1 — Pancreatitis Is Inflammation, Not Simply Enzyme Elevation

Pancreatitis involves inflammatory injury within and around the exocrine pancreas. Premature activation of digestive zymogens and inflammatory pathways can injure pancreatic tissue and surrounding fat.

Clinical severity ranges from subclinical disease to vomiting, pain, shock and multiorgan complications.

Part 2 — Routine Lipase Activity Has Biological Sources Beyond the Pancreas

Merck notes that many traditional serum lipase activity assays also measure non-pancreatic lipases. Renal, gastrointestinal, hepatic, neoplastic and drug-related influences can alter total lipase activity.

This is why an assay name such as “lipase” is incomplete unless its analytical target is known.

Secondary Deepening — PLI Is Designed to Measure Pancreatic Lipase

Pancreatic lipase immunoreactivity uses antibodies directed against pancreatic lipase and is species-specific in dogs and cats. Merck describes PLI as the most specific serum diagnostic test for pancreatitis in these species.

Part 3 — Pancreas-Specific Does Not Mean Disease-Specific

A pancreas-specific analyte tells us where the measured molecule came from. It does not guarantee that every increase represents clinically important pancreatitis.

Test performance changes with disease severity, timing and the population tested. Borderline results therefore require clinical and imaging context.

Part 4 — Ultrasound Adds Structure

Ultrasonography may show an enlarged or hypoechoic pancreas, surrounding hyperechoic fat, fluid or biliary/gastrointestinal changes. But sensitivity depends on operator experience, patient anatomy and disease stage.

A normal ultrasound therefore does not eliminate every pancreatitis case, while an abnormal pancreas-like appearance still needs clinical correlation.

Part 5 — Vomiting Is a Network Symptom

Vomiting can arise from stomach or intestinal disease, obstruction, kidney disease, liver disease, toxins, endocrine disease, vestibular/central causes and pancreatitis.

The pancreas enters the differential because of pattern and evidence—not because vomiting belongs to the pancreas.

Part 6 — Exocrine Pancreatic Insufficiency Is a Different Failure

EPI occurs when exocrine pancreatic secretory capacity falls so far that normal digestion cannot be maintained. Typical patterns include weight loss despite appetite, poor body condition, voluminous stool or diarrhoea and nutrient deficiencies.

Texas A&M notes that the pancreas has substantial functional reserve: clinical EPI appears only after a very large reduction in exocrine secretory capacity.

Part 7 — TLI Asks About Exocrine Pancreatic Tissue Function

Trypsin-like immunoreactivity measures circulating trypsinogen/trypsin derived from pancreatic acinar tissue using species-specific assays.

Very low TLI strongly supports EPI because there is too little functional acinar tissue producing the expected zymogen signal.

Texas A&M updated its canine TLI interpretation in 2024 after detecting an assay shift, illustrating why current laboratory-specific decision thresholds matter.

JC Deepening — Inflammation and Functional Reserve Are Different Axes

Pancreatitis asks whether exocrine pancreatic tissue is inflamed. EPI asks whether enough functional exocrine tissue remains to support digestion.

Chronic pancreatitis can eventually reduce exocrine reserve enough to cause EPI, but many pancreatitis patients never develop EPI and some EPI cases arise from other mechanisms.

Part 8 — Species Differences Matter

Dogs and cats differ in clinical presentation and in the value of older pancreatic tests. Merck notes that routine serum amylase and lipase have little value for feline pancreatitis diagnosis.

This reinforces a branch rule: diagnostic performance must be calibrated to species, assay and disease state.

Part 9 — Concurrent Disease Is Common, Especially in Cats

Pancreatic inflammation can coexist with intestinal, biliary, endocrine or renal disease. That makes a positive PLI result potentially real and still incomplete as an explanation for all clinical signs.

The RFE therefore asks whether the pancreas is the sole driver, one contributor, or an incidental coexisting process.

How Do We Know?

Veterinary pancreatic diagnostics are validated by comparing assays with clinical cohorts, imaging, histopathology where available, treatment response and longitudinal outcome. Texas A&M’s GI Laboratory has been central to developing and recalibrating species-specific PLI and TLI assays.

Observation vs Inference

Unfamiliar Transfer

Dog A vomits for one day and has a high nonspecific lipase activity but normal pancreas-specific PLI and no supportive ultrasound findings. Dog B has chronic weight loss, bulky stool and very low TLI.

A weak learner labels both “pancreas disease” and stops. A strong RFE answer recognises that Dog A has weak evidence for pancreatitis while Dog B has a different exocrine failure—loss of digestive secretory capacity.
